Note: we must have a completed medical release form in order for you to travel with us on the bus.  There is a place for the medical release form to be notarized. Some choose not to notarize the form.  BUT, if you choose NOT to notarize the form and the hospital delays in treating your child because the form is NOT notarized, that is YOUR CHOICE.  SYD has a notary and can notarize your forms. They must be signed in the presence of the notary.
